General Policies and Information

Acupuncture and Oriental MedicineFirst-time Patients:

New clients must schedule a Comprehensive Case review or a Comprehensive Case Review with Acupuncture.
Online forms will be emailed to you upon scheduling first appointment. Please be sure to complete the forms within 24 hours as the link will expire. Please call if you have problems with these forms so we can resend the forms. Your first appointment will include a comprehensive case review and evaluation. We will go over your past and current health history, in addition to discussing what you hope to achieve. 

Cancellation or Missed Appointment Policy:
Your time and ours is valuable. Please allow 24 hours' notice for cancellation of appointments. This gives us time to schedule someone else in your cancelled time slot. Appointments cancelled without 24 hours' notice or missed will be billed at the full appointment rate.

Personal Conduct:
ANY overt or implied threats, sexual remarks, misconduct, or advances will result in the immediate discontinuation of the appointment. The full price for the service will still be owed. We reserve the right to refuse service to anyone.

Insurance Information:
The clinic is not a provider within any insurance network. Upon request, we can provide you with paperwork to submit to your insurance company or HSA for possible reimbursement. Please note that not all health plans cover acupuncture, and deductibles may apply. We do no guarantee reimbursement or coverage.

Your Privacy is Important:
Patient information and records are private and confidential, as outlined by HIPAA regulations.
